Through an integrative, patient-centered approach, Weller Health Transitions employees and the affiliates strive to elevate the standard of care in Dayton nursing homes via improved communications with families and coordination of care with facility nurses, therapists, social workers, dietitians, administrators, local hospital care teams and community physicians.
We have committed ourselves to educating our patients in the perpetual search for understanding the aging mind and body. We provide advice on local resources that benefit our elderly population. We encourage Advanced Care Planning and counseling.
